Program Type: Community-Based

To Make Referrals: Services can be accessed through community referrals such as the county assessment team, shelters, respite placement, hospitals, family members, churches, police, community members, etc. Referrals can be made by phone or sending a referral form. Services can also be accessed through ÌÀÍ·Ìõapp’s Outreach & Engagement 3 team.

Who We Serve:

  • Resident of Stanislaus County

  • Transitional Age Youth (TAY) ages 18-25 age with serious mental illness

  • Individuals who are homeless or at risk of homelessness, high users of crisis-based services, individuals with high utilization rates of hospitalization services, individuals with co-occurring substance use disorders, and unserved and underserved ethnic/ cultural populations

Hours of Operation:

  • Monday-Friday: 8:00 a.m. to 5:00 p.m.

  • 24/7 crisis services available

We believe challenges can be an opportunity for growth and that everyone has strengths and gifts which support their personal wellness. Our multidisciplinary staff provides a wide range of services and supports in a comfortable and welcoming environment.

Our program is based on the Assertive Community Treatment (ACT) model which offers a range of services that are comprehensive, culturally competent, and flexible. Our wraparound approach includes formal therapeutic interventions and informal supports like family involvement and advocacy and linkages to community-based organizations, and resources to give you a complete and comprehensive recovery.
