Essentials At A Glance

Program Type: Community-Based

To Make Referrals: Referrals are made through multiple sources, including ÌÀÍ·Ìõapp County Health Care Agency and community organizations as well as walk-ins.

Who We Serve: Residents of ÌÀÍ·Ìõapp County, aged 18 and older, who have been diagnosed with a serious mental illness and who may be:

  • Homeless or at-risk of homelessness

  • Have a co-occurring substance use disorder

  • At risk for hospitalization or institutionalization

  • Involved in the criminal justice system

  • Transitioning from institutional care

  • Not successful with traditional mental health care

Hours of Operation:

  • Mon, Wed, Fri & Sat: 8:00 a.m. to 5:00 p.m.

  • Tues & Thurs: 8:00 a.m. to 7:00 p.m.

  • On-call staff are available 24 hours a day

About MISSION POSSIBLE: The MISSION POSSIBLE team was developed to provide specialized, recovery-focused services in the community using mobile vehicles to serve individuals with serious mental illness who are homeless in ÌÀÍ·Ìõapp County, CA.

Our model of care focuses on multidisciplinary teams that will build relationships with individuals leading to successful linkage with ongoing mental health services, physical healthcare, and housing. The Outreach Team consists of two case managers and a peer support specialist. The Treatment Team has a nurse practitioner, two case managers, and a peer support specialist. We work with the ÌÀÍ·Ìõapp County Health Care Agency Outreach and Engagement Team to provide regularly scheduled visits within the county and as permitted by local jurisdictions so that clients come to know when to expect us.

The Mobile Outreach and Treatment Services are funded by the Homeless Mentally Ill Outreach and Treatment grant.

We believe recovery starts from within, and that our job is to do whatever it takes to provide the support needed on your recovery journey. We have a multidisciplinary team that includes peer specialists, a psychiatrist, a nurse, a licensed team lead as well as a masters-level team lead, and case managers with experience working in both mental health and co-occurring substance use, who are all here to promote a program culture where resilience and hope can flourish.

Your recovery plan starts with you. We use transformational care planning, a recovery-oriented treatment planning method to develop an individualized recovery plan to guide your work in the program. We assist you by giving you the appropriate skills where you will better be able to manage your conditions more effectively, monitor your progress toward your goals, and adjust services to fit your changing needs.
